Care Clinics Inc. has been providing state licensed outpatient alcohol, drug and DUI counseling and DUI classes since 1981. We are state licensed by the Illinois Department of Human Services - Division of Substance Use, Prevention and Recovery (DSUPR) to provide DUI Evaluations, DUI risk reduction education, outpatient substance abuse treatment, and intensive outpatient (IOP) substance dependency treatment. We work with you on a personalized level to make sure you get what you need.

A DUI evaluation is required when loss of driving privileges are due to a DUI arrest. Before you can begin any DUI classes or treatment, you must complete a drug and alcohol evaluation.
Evaluations can be available within 2 working days and if needed, the evaluation can be available for same day pick up with an additional charge of $25. This service is perfect for an out-of-state client flying in for a court appointment the following day.
What to Expect During the Process
DUI Evaluations are one-on-one appointments with a member of our clinical staff. These appointments are done virtually or in-person and typically take 1 to 1.5 hours. At the time of evaluation, you are required to provide a complete driving record from the state issued or if in Illinois a "court purposes" driving record or "abstract." You are also required to provide documentation of drug or alcohol testing or proof that you refused/failed to complete testing on the night of arrest.
We are happy to walk you through this when you make an appointment.
If your arrest was due to DUI there are five risk classifications which you can be assigned. Each risk level has state mandatory criteria. This is what we are going to determine during your evaluation.
Care Clinics Inc. has been assisting those individuals who have lost their driving privileges due to DUI charges which occurred in the State of Illinois as well as out of state. We are highly experienced with handling misdemeanor, felony, and fatal DUI revocations.
The Secretary of State Investigative evaluation ($150) is utilized when the loss of driving privileges is not due to a DUI arrest. If your arrest was due to Zero Tolerance, Underage Drinking/Possession, Vehicle Manslaughter, or a felony conviction you will need an “Investigative Evaluation.” This evaluation is more narrative in nature and does require a corroborative interview with someone who knows you and your substance use history. The recommendations can include “no recommendation,” alcohol/drug educational class, alcohol/drug counseling, and/or “other” recommendation.
The Secretary of State Out of State Application ($200) is utilized when the loss of driving privileges is due to a DUI arrest and you live in another state.
Care Clinics Inc. can also provide treatment needs assessments, evaluation updates, continuing care status reports, and referrals to attorneys that specialize in driver’s license reinstatement.
What to Expect During the Process
This evaluation is utilized when a client wants to determine if they have an alcohol or drug problem. It can also be utilized when requested by court order, probation department, parole agent, employer/EAP, or family member.
